Group Adult Foster Care
Group Adult Foster Care (GAFC) is a long-term care community option that provides elderly and/or disabled clients with daily personal care assistance.
Overview
The Group Adult Foster Care (GAFC) program is a MassHealth program that provides personal care services in either a community or assisted living setting to individuals who are elderly and/or disabled. GAFC provides assistance with Activities of Daily Living (ADLs), nursing oversight and care management.
It is designed to serve residents with the goal of providing support services they may need to help them remain living in their own homes instead of moving to institutional placement. Individuals must meet the criteria and guidelines set forth by MassHealth:
Requirements:
- Have MassHealth Standard
- Be 22 years of age or older
- Need HANDS ON daily assistance with personal care tasks such as:
- Bathing
- Dressing
- Toileting
- Transfers
- Ambulation
- Eating
